Important Notice on HIPAA Privacy Practices for Our Nutritional Counseling Program

We are committed to promoting wellness and health awareness through mindful nutrition at SABIA NUTRITION. If you choose to participate in any of our Nutritional Counseling Programs, this notice explains how your medical information may be used and shared, and how you can access it. Please read it carefully.

Your Privacy Matters to Us
SABIA NUTRITION takes your health information privacy very seriously. Under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”), we are required to provide you with this Notice of Privacy Practices (“HIPAA Privacy Notice” or “Notice”) and abide by its terms. This document outlines how we may use and disclose your Protected Health Information (“PHI”) in connection with our Nutritional Counseling Services, which include “Special Diets,” “Advanced 360 Program,” “Nutritional Counseling Services,” and other related programs.

Please Note: SABIA NUTRITION does not collect or store protected health information (PHI) outside of our Nutritional Counseling Programs. If you have never participated in any of our programs, we have not collected or accessed your PHI.


How We Use and Share Your PHI

We may use and disclose your PHI for your nutritional care, payment processing, and operational support of our programs. In compliance with HIPAA, we will only use or share the minimum necessary PHI to fulfill our services.

Treatment and Nutritional Support

We use PHI to provide, coordinate, and manage your nutritional care. This includes maintaining secure records of information you provide through questionnaires, consultations with our dietitians or wellness coaches, and personalized nutrition plans.

Payment Processing

We may use and disclose PHI to process payments for our nutritional counseling services.

Operational Purposes

We use PHI to evaluate and improve our services, monitor performance, and conduct internal business operations.

HIPAA-Approved Disclosures

We may disclose your PHI without additional authorization in the following situations:

  • Legal Compliance: To meet federal, state, or local legal requirements.

  • Public Health and Safety: For disease tracking, reporting abuse or neglect, or responding to public health risks.

  • Regulatory Oversight: For audits, inspections, and government investigations.

  • Law Enforcement Requests: To comply with court orders, subpoenas, or legal processes.

  • Product Safety Monitoring: To report food-related safety concerns to regulatory agencies.

  • Workplace Injury & Compensation: To comply with workers’ compensation laws.

  • Care Coordination: To communicate with healthcare providers or designated family members involved in your care.

  • Third-Party Service Providers: To allow vendors assisting us in operations to access PHI under strict security agreements.

We will require your written authorization for any other use or disclosure not covered in this Notice. You may revoke this authorization anytime, though it will not affect past disclosures.


Your HIPAA Rights

You have specific rights regarding your PHI:

  • Access & Copies: You can request an electronic or paper copy of your PHI. In limited cases, we may deny access but will provide a written explanation.

  • Amendments: If you believe your PHI is incorrect, you may request corrections. If denied, we will provide a written explanation.

  • Accounting of Disclosures: You can request a list of disclosures made in the past six years, except those related to treatment, payment, or internal operations.

  • Restrictions on Disclosure: You may request limits on how your PHI is shared, though we are not required to agree unless the law mandates disclosure.

  • Confidential Communication: You can request contact at a specific phone number, email, or mailing address.

  • Paper Copy of This Notice: You may request a physical copy of this Notice at any time.

  • Designated Representative: If you have authorized a medical power of attorney or legal guardian, they may exercise your HIPAA rights on your behalf.


Our Responsibilities

Under HIPAA, we are required to:

  • Maintain the privacy and security of your PHI.

  • Provide this Notice outlining our privacy obligations.

  • Comply with the current terms of this Notice.

  • Notify you in case of a security breach that affects your PHI.

Some states have additional privacy laws that may apply to your PHI.


Policy Updates

We may periodically update this Notice to reflect changes in regulations or our business practices. The revised Notice will apply to all PHI, including past records. The latest version is on our website, and your continued use of our services implies your acceptance of any updates.
